What Concierge Membership Includes
As a concierge member, you receive:
- Extended appointment times
- Same-day or next-day scheduling when available
- Direct communication with your care team
- Response within 6 business hours during normal business hours once established
- Personalized preventive care planning
- Chronic disease management support
- Care coordination and follow-up
- Acute sick visits via telehealth or home visits when appropriate
- In-home flu, RSV, COVID, and strep testing
- Nutrition counseling and lifestyle coaching
This model allows for deeper conversations, proactive care planning, and greater continuity over time.
